How do I file a life insurance claim?
If an insured has passed away, either the beneficiary of a life insurance policy or someone else who was close to the insured can start the claims process with us. To get started, please contact Fabric by sending us a support message.
Who can file the claim?
Typically, the beneficiary of the policy would initiate the claim. That said, the person who files the claim doesn’t necessarily have to be a beneficiary, as long as they can answer basic questions about the insured’s personal information.
What information do you need?
To start the claim, we mainly need information about the person reporting the claim and enough details to locate the insured in our system, including:
Full name of the insured
Any email addresses associated with the insured
Name, phone, and email of the person reporting the claim
Other details, like address, date of birth, Social Security number, policy number, or beneficiary information, are helpful if available, but we can fill these in once we locate the insured in our system.
We may also ask a few basic questions about the circumstances of the death, such as:
Date of death
Whether it was accidental
Whether it occurred in the United States
These details help us prepare the claim, but they are not required to start the process.
What happens next?
We’ll guide you through completing a short initial death claim notification. After that, the insurance company will follow up with the next steps and any additional forms required to complete the claim.

Do I need a death certificate?
No. A death certificate or any other documents are not required to start the claim. Any documents needed will be requested later as part of the official claims packet.
Who is the insurer of the policy?
Policies purchased before September 3, 2022: Vantis Life Insurance Company
Policies purchased on or after September 3, 2022: Western-Southern Life Assurance Company
When should I file the claim?
Claims should be filed within 20 days of the insured’s passing, or as soon as reasonably possible.
How quickly are claims paid?
Policies purchased before September 3, 2022:
For Vantis Life Insurance Company policies, if the claim is received in good order, payment of the death benefit will be processed within three business days of receipt. Claims that are under investigation cannot be paid until the investigation is complete.
Policies purchased after September 3rd, 2022: Claims are reviewed by Western-Southern Life Assurance company and will be paid out as quickly as possible. Claims within the contestability period will be reviewed and cannot be paid until the review is complete.
