How do I file a claim?

Someone who was close to the insured person will need to start the claims process with Fabric.

In the event that an insured passes away, either the beneficiary of a life insurance policy or someone else who was close to the insured, will need to first contact Fabric by sending us a support message. The details that are helpful to have readily available at this stage are the insured’s name, DOB, policy number (if you have it available), and any e-mail addresses that they had.

We’ll walk you through the process of filing a claim, which includes the completion of a brief questionnaire, called an initial death claim notification. This is used to gather necessary information about the insured, the beneficiary, and the policy.

Once that’s done, the insurance company will contact you with further instructions and subsequent claims forms to complete. For policies purchased before September 3rd, 2022, the insurer is Vantis Life Insurance Company. For policies purchased on or after September 3rd, 2022, the insurer is Western-Southern Life Assurance Company.

The person who files the claim doesn’t necessarily have to be a beneficiary of the policy, as long as the person is able to answer questions about the insured’s personal information within the questionnaire.

If the person reporting the claim has a copy of the insured’s death certificate, this could be sent to us electronically, although it’s not required at this point in the claims process.

The claim should be filed within twenty days of the insured’s death, or as soon as reasonably possible.
