How do I file a life insurance claim?

If an insured has passed away, you can start a claim by completing our online form:

You can submit the form whether you are the beneficiary or someone assisting with the claim.

Who can file the claim?

Typically, the beneficiary of the policy initiates the claim. However, anyone with basic information about the insured can submit the form to begin the process.

What information do you need?

To get started, we ask for:

  • Information about the person reporting the claim (name, phone, email)

  • Basic details about the insured, such as:

    • Full name

    • Date of birth

    • Date of death

If available, it’s helpful to include:

  • Policy number

  • Email address associated with the policy

  • Last four digits of the Social Security number

We may also ask a few additional questions, such as:

  • Whether the death was accidental

  • Whether it occurred outside the United States

If you don’t have all the information, you can still submit the form. These details help us prepare the claim, but they are not required to start the process.

What happens after I submit the form?

Once your submission is received:

  1. Our team will review the information provided

  2. We will gather any necessary policy details

  3. The claim will be initiated with the insurance company

  4. You will receive a confirmation email with next steps

Do I need a death certificate?

No. A death certificate or other documents are not required to submit the form.
If documentation is needed, it will be requested later as part of the official claims process.

Who is the insurer of the policy?

  • Policies purchased before September 3, 2022: Vantis Life Insurance Company

  • Policies purchased on or after September 3, 2022: Western-Southern Life Assurance Company

When should I file the claim?

Claims should be filed within 20 days of the insured’s passing, or as soon as reasonably possible.

How quickly are claims paid?

Policies purchased before September 3, 2022:
For Vantis Life Insurance Company policies, if the claim is received in good order, payment of the death benefit will be processed within three business days of receipt. Claims that are under investigation cannot be paid until the investigation is complete.

Policies purchased after September 3rd, 2022: Claims are reviewed by Western-Southern Life Assurance company and will be paid out as quickly as possible. Claims within the contestability period will be reviewed and cannot be paid until the review is complete.
